Do you have goals for your future medical needs? Do you know your medical rights? Do you have wishes for end of life care? These questions and more will be answered during a free community lunch and presentation at 11:30 a.m. on Monday, Nov. 5 at The Chapel behind Adventist Health Medical Office – Hanford, 1025 N. Douty Street.

Palliative Care Nurse Navigator Kerry Cromwell, from Adventist Health Hanford, will share with elderly patients the importance of an Advanced Health Care Directive. It’s a form that specifies a patient’s wishes and goals for the end of life care. Cromwell will even show participants, their loved ones and care providers how to fill out the form.
